I am an ignorant American. (Surprise, surprise) Please explain O-level. I've always wondered what it means.

It is "Ordinary" level examination, as opposed to "Advanced" (A) Level.
They are the examinations taken at the end of your school career.
Around 15 you take "O" levels and then at 16/17 you can take "A" levels, results of which determine your entry to University.

They are no longer called "O" levels however, that was back in the 60's and 70's, they are now called GCSE's (General Certificates of Education). Skinny is obvioulsy one of the "old school".
